Discrepant post-filter ionized calcium concentrations by 2 common gas analyzers in continuous renal replacement therapy using regional citrate anticoagulation: another piece of the puzzle

نویسندگان

چکیده

During continuous veno-venous hemodialysis with regional citrate anticoagulation, ionized calcium (iCa) measurements in pre- and post-filter samples are used to adjust flows. Post-filter iCa target concentrations within a low range (0.25–0.34 mmol/L), blood gas analyzers (BGAs) mainly calibrated for physiological values (1.12–1.20 mmol/L). Bias between BGAs these low-value leads errors adjusting the dose,1,2 yet current anticoagulation protocol is worldwide despite lack of standardization.

Discrepant post filter ionized calcium concentrations by common blood gas analyzers in CRRT using regional citrate anticoagulation

INTRODUCTION Ionized calcium (iCa) concentration is often used in critical care and measured using blood gas analyzers at the point of care. Controlling and adjusting regional citrate anticoagulation (RCA) for continuous renal replacement therapy (CRRT) involves measuring the iCa concentration in two samples: systemic with physiological iCa concentrations and post filter samples with very low i...

Optimizing citrate dose for regional anticoagulation in continuous renal replacement therapy: measuring citrate concentrations instead of ionized calcium?

Regular measurement of systemic and post-filter ionized calcium (iCa) concentrations is imperative to correctly handle regional citrate anticoagulation dose during continuous renal replacement therapy (CRRT). Keeping post-filter iCa within a tight range guarantees optimal circuit function and enhances filter life span [1, 2], whereas a decrease in plasma iCa, with subsequent elevation of the to...

Ionized calcium measurements during regional citrate anticoagulation in CRRT: we need better blood gas analyzers.

Schwarzer et al. [1] showed discrepant postfilter ionized calcium concentrations [iCa] when using different blood gas analyzers (BGAs) and called for a change of the Fresenius regional citrate anticoagulation (RCA) protocol to ensure patient safety. Of note, precision of the BGAs was not tested. In our opinion, the key message of their study is that commercially available BGAs are not accurate ...

Ionized Magnesium and Regional Citrate Anticoagulation for Continuous Renal Replacement Therapy.

BACKGROUND The regional citrate anticoagulation (RCA) induces changes in total (Catot) and ionized (Ca2+) calcium. As of now, we do not have much information about parallel changes of total (Mgtot) and ionized (Mg2+) magnesium. METHODS The authors compared changes of Mg2+ and Mgtot with changes of Ca2+ and Catot in 32 critically ill patients on 4% trisodium citrate (4% TSC) with calcium-free ...
